____ Budget Explanation (No indirect costs, equipment rarely allowed--must be justified, full detail on travel)
____ Biographical Sketches (1 or 2 pages) and a list of major publications during the last five years
____ Form D - Conflict of Interest (for each investigator)
____ Form E - Cost Sharing Certification (Use official corporate name, full mailing address)
____ Cost Sharing Committment (letter from company sponsor acknowledging the following:  CIT involvement in the project, actual amount of company sponsor contribution, project manager, anticipated benefits to the company, agreement to provide economic impact data and acceptance of terms outlined in the industry agreement.)
____ Form F - Company Sponsor Information
____ Partnership Plan (1 page maximum)
____ Commercialization Plan (4 page maximum)
____ Form G - Economic Impact
____ Distribution List
