Students under age 18 must have their parent’s permission to receive vaccines. Please ask your parent or guardian to fill out an immunization consent form and fax it to the number on the form (410-910-1720) or bring it with you on October 17th. Click HERE for the form.

If you would like additional information about the vaccines available at the Immunization Clinic, you may read the Vaccine Information Sheets, available from the Centers for Disease control (CDC).

Don’t be BLOCKED from spring registration! Get your pre-entrance immunizations completed on the same day. Please bring your student health vaccination notice letter with you so we will know what you need.
