Facilities Management

Purpose
To request and document the manufacture of University keys under
the University's Master Keyed System.
Instructions
See back of form for instructions.
Procedure
All key blanks and materials are furnished by Facilities
Management. Submit a Key Request Form when requesting keys.
USE A DIFFERENT FORM FOR EACH TYPE OF KEY. Additional copies of
the same key may be requested on the same form.
Key transfers (non-master key) are permitted. Please inform
Facilities Management immediately.
Responsibility Action
Requesting Department 1. Completes request form. If
needed,contacts Locksmith for
assistance at ext. 982-4676.
2. Forwards forms to FM for change keys
only.
3. Forwards form to the appropriate
vice-president (master/submaster
keys only).
Approving Authority 4. Reviews for authorization and
forwards to University Chief of
Police.
Responsibility Action
University 5. Approves or disapproves
Chief of Police and forwards to FM.
Facilities Management 6. Performs required task.
7. Informs department, keys are
ready for pickup or delivery.
8. Provides Facilities Management Key
Issue/Return Destruction Record,
Form 2-93.
Person Receiving Key 9. Signs Facilities Management Key
Issue/Return Receipt/Destruction,
Form 2-93.

NOTE: A master key must be picked up personally by the person
responsible for its use. Other keys may be picked up by the
designated department official responsible for the custody of
keys. Master Keys are not transferrable.
NOTE: Facilities Management charges for delivery of keys,
locks, etc.
