Requisition Form
Obtain Blank Forms From: Blank forms may be obtained from the
Printing Office or by calling 924-
7186.
Forward Completed Forms To: Send or deliver the top three copies
to the Printing Office with a sample
or printing layout, if available.
Retain fourth (goldenrod) copy for
your records.
Purpose
To requisition graphic design, typesetting and printing services
from the University Printing Office.
Instructions
1. Show the departmental name and messenger mail address of the
department TO BE CHARGED for services requested.
2. Indicate the LOCATION the work should be delivered to when
completed, including the name of any specific individual who
should receive it.
3. Print the name of the responsible individual to be contacted
in the event of questions or difficulties, and the phone
number where he/she can be reached. The requestor should
sign the requisition to the right.
NOTE: If the requesting department requires additional approvals,
these spaces should also be used for these signatures.
4. Indicate the date the requisition will be sent or delivered
to the Printing Office.
5. Indicate the complete University Fund Account Code, including
object Code.
6. Indicate the desired delivery deadline. Fifteen working days
is normally required for routine printing jobs, and is
requested. Please endeavor to plan accordingly. Contact our
Customer Service staff for special requirements.
7. Describe the work to be produced as completely as possible.
Please DO NOT USE "SEE ATTACHED" or similar phrase. CALL THE
PRINTING OFFICE IF ASSISTANCE IS NEEDED IN DRAFTING A WORK
DESCRIPTION.
8. Enter the quantity of finished, printed pieces desired.
Padded materials are printed as single sheets and then
padded, and the quantity ordered should be for the number of
sheets desired. Multiple-part forms should be ordered as
sets, e.g., 5,000 3-part sets.
9. Check the box that most appropriately describes the form of
the finished product.
10. Show the finished size of the product, in inches.
11. Check the appropriate box.
12. Indicate your proofing requirements, if any, and to whom they
should be sent.
13. Fill in this non-shaded area as applicable or necessary to
convey your needs and instructions.
14. You may use this space for additional instructions of
clarifications.
Please call the Printing Office for an appointment or telephone
discussion of your requirements if you do not feel you can
adequately convey complete and concise instructions on the
requisition form. The Customer Service staff will provide
assistance as needed.
NOTE: Any requisition submitted without a valid charge code,
adequate instructions, samples or other information needed to
produce your printing work, is subject to delays.

Codes, or with cash. Individuals and private organizations must
pay cash for services.
