NOTE: ALL FORMS MUST BE TYPED. HANDWRITTEN FORMS WILL BE RETURNED
TO THE REQUESTOR.
Purpose
This form is used ONLY to request new telephone service from Siemens
or changes to existing Siemens Service.
See also Procedure 7-30, "Preparation of Telephone Service Request-
Embarq."
Instructions
The numbered items on the sample form should be completed as
follows:
1. Enter the requestor's name.
2. Enter the requestor's department.
3. Enter the requestor's messenger mail address.
4. Enter the location(s) where work will actually be performed.
5. Identify the work to be performed by checking the appropriate
line.
NEW LINE - Used to request a new 924 or 982 number. Unless
the "jack only" entry is marked, a new telephone instrument
will be installed.
NEW EXTENSION TO EXISTING NUMBER - Used to request an
additional telephone instrument for a 924 or 982 number
already in service.
CHANGE - Used primarily to request modifications to software
features associated with a 924 or 982 number already in
service. The features that can be added/changed/removed are:
Class of Service
COM Group
Hunt Group
Distribution Group
Pickup Group
System Forwarding
RELOCATION - Used when a telephone line is to be moved from
one location to another, or if equipment attached to a
telephone (e.g., a button and buzzer arrangement) is to be
moved from one instrument to another.
RENUMBER - Used to request number swaps between two or more
existing instruments. Also used to request a different
number for an existing instrument.
REMOVAL - Used to request discontinuation of service for an
instrument and/or telephone number.
PHONEMAIL - Used to request the addition/change/removal of
PhoneMail service to/from an existing telephone number.
ACD - Used to request the addition/change/removal of
Automatic Call Distribution Service to/from one or more
telephone numbers.
OTHER--Used to request long handset and base cords and
special items of equipment (e.g., including, but not limited
to, beehive lights, button and buzzer arrangements, busy lamp
fields, chimes, confidencers, and amplified handsets).
6. Identify the existing 243, 924, or 982 number for which work
is being requested.
7. a. Identify the type of telephone or desktop product to be
installed or on which work is to be done.
b. Identify the inventory numbers of the telephone or
desktop product. The inventory number is on the
underside of a desk telephone and on the bottom edge of
a wall telephone.
8. If the 243, 924, or 982 number is to be part of a COM group,
list other telephone numbers that will be in the same COM
group.
9. Enter the class of service which exists for/will be assigned
to the 243, 924, or 982 number on which work is to be
performed. Telephone counselors have a list of the two digit
class of service codes. Siemens (924-8600) also has a list of
the codes and the corresponding level of service available
with each.
10. If the telephone number is to be system forwarded, check the
condition(s) which will apply. Under "extension" list the
five digit telephone number to which calls will be forwarded.
Calls can forward to one extension for a "no" answer
condition and to a different extension for a "busy"
condition.
11. If the telephone number is to be placed in either a hunt or
distribution group, list other telephone numbers that will be
assigned to the same group. The ringing order for telephone
numbers in the group should be identified in the "explanation
of other" section (item 16).
12. If the telephone number is to be placed in a pickup group,
list other telephone numbers that will be assigned to the
same group.
13. If the telephone number is to be placed in an Automatic Call
Distribution (ACD) Group, list other telephone numbers that
will be assigned to the same group.
14. Identify the current location (room and building) of the line
or equipment for which work is being requested. Leave this
entry blank if requesting new service.
15. If a line or instrument is being relocated, identify the room
and building to which the service will be moved.
16. Enter more detailed information for items 5 - 15 as required.
Attach additional pages if necessary.
17. Identify the contact person for additional information and/or
to provide access to the location where work is to be
performed.
18. Identify the telephone number of the contact individual
listed in item 17.
19. Contact Communication Services to obtain both the recurring
and one-time charges associated with the work requested.
20. Identify the 11-digit account code to which the cost of
service is to be billed.
21. Indicate the date by which the requested service is to be
performed.
22. Either state the account listed in item 20 has sufficient
funds to pay for the requested work or that additional money
must be obtained.
23. Sign the funds availability certification.
24. Leave blank.
Note: Once approved, Communication Services will return a
copy of the form to the requestor. The form will contain the
telephone service request number in the upper right hand
corner. Please use this number when referring to this
service request.
