Purpose

This form is used by departments to report any incidents
resulting in injury, damage, or loss of University property or
equipment.

ALL THEFTS MUST BE REPORTED TO THE UNIVERSITY POLICE DEPARTMENT
IMMEDIATELY. AN INSURANCE INCIDENT FORM (PROCEDURE 7-80) MUST ALSO
BE SENT TO RISK MANAGEMENT WITHIN 24 HOURS OF DISCOVERY.  NOTIFY 
PROPERTY ACCOUNTING OF THE PERTINENT FACTS FOR PROPERTY REPORTING 
PURPOSES.
