Obtain Blank Forms From:       Fiscal Administrators, pages 8.9.9 -
                               8.9.10.  Makes copies as needed.

Forward Completed Forms To:    Completed forms should be attached
                               to the proposal approval sheets and
                               forwarded to the Research
                               Administrator for processing.

Purpose

To certify that lower tier subcontractors have not been or are
being debarred or suspended from participating in federal financial
assistance programs and activities.

Form Completion Instructions

Self-explanatory.

NOTE:  This form must be completed by the lower tier subcontractor
and should be attached to the proposal approval sheet at the time
