Form:                          Drug Study Questionnaire

Obtain Blank Forms From:       Fiscal Administrators, page 8.9.16,
                               Forms Packet.  Make copies as
                               needed.

Forward Completed Forms To:    Completed forms should be attached
                               to the proposal approval sheet and
                               forwarded to the research
                               administrator for processing.

Purpose

To determine whether the grant or contract is related to the
University's tax exempt status.


Background

The University, as a non-profit entity, is subject to reporting to
the Internal Revenue Service (IRS) certain activities considered
unrelated to the tax exempt status of the University.  Determining
whether an activity is unrelated can be complex.  The drug study
questionnaire was developed to more fully comply with IRS reporting
requirements.

The questionnaire must be completed for any clinical grants or
contracts involving:

      Corporate or industry sponsors, AND

      The study, testing, evaluation, or research of drugs (whether
      or not such drugs are FDA approved).

NOTE: The questionnaire is required solely to improve the
      University's reporting compliance with the IRS.  It will have
      no bearing on whether the contract or grant is accepted or
      rejected.
