2012-2013 Academic Year

Below you will find financial aid forms for the 2012-2013 academic year. Please choose the correct form(s) for your needs and return to our office.

Appeal for Reconsideration -- (Completed by parent or student) This form should be submitted when making an appeal for reconsideration of special circumstances (e.g., loss of or reduced employment, excessive medical/dental expenses, dependency status change)

Asset Information Request Form -- (Completed by a Parent or Student) This form should be submitted when verifying Asset Information.

Authorization to Release Financial Information -- (Completed by the Student) Allow University personnel to discuss student financial records with parties other than the student (e.g. parents). Without this form on file, our staff will be unable to offer important assistance to parents and others who have questions about the student account. We recommend all students complete this form.

Child Support Paid Form -- (Competed by the Student and the Payer) This form is used to confirm the amount of child support paid by a parent or step-parent in 2011. For independent students, a parent may be the student or if married, the student’s spouse.

College Enrollment Verification Form -- (Completed by Dependent or Independent Students) This form should be submitted when verifying the college enrollment of the student's family members.

Consent to Obtain Credit Check -- (To be completed by the Parent) This form is used to grant consent to the University to obtain a credit report to determine eligibility for a Direct Parent PLUS loan.

Dependent Document of Support Form -- (Completed by a Dependent or Independent Student) Use this to document the support given to additional members of your household (e.g. grandparents, graduate students).

Direct Parent PLUS Loan Application Form -- (Completed by parent of a dependent student) This form must be submitted when applying for a PLUS loan for Undergraduate Students. PLEASE NOTE: In addition to this form, the parent must complete an application and Master Promissory Note at studentloans.gov, after that application becomes available May 1, 2012.

Faculty and Staff Scholarship and Sandridge Scholarship Application -- (Completed by the student or parent/grandparent who is a University employee)

Family Income and Expense Form -- (Complete by the Parent of a Dependent Student or by an Independent Student) Detailed list of monthly income and expenses for 2011.

Financial Aid Cancellation Request Form -- (Completed by the Student) This form should be submitted to request a partial or a complete cancellation of financial aid.

Financial Aid Change Form - Decrease -- (Completed by the Student) This form should be submitted when reporting and/or requesting a decrease that will affect a student's financial aid award.

Financial Aid Change Form - Increase -- (Completed by the Student) This form should be submitted when reporting and/or requesting an increase that will affect a student's financial aid award.

Graduate Verification Form -- (Completed by a Graduate student) This form must be submitted when a Graduate student has been selected for federal verification.

Health Insurance Expense Request Form -- (Completed by the Student) Use this form to request an increase to your student budget to accommodate possible additional loan amounts to cover the cost of health insurance.

Household Size Form -- (Completed by Dependent or Independent Students) This form should be submitted when verifying the number of dependents within a household. Generally, the Household Size Form is requested when the number reported in the household size and/or number in college on the Free Application for Federal Student Aid (FAFSA) and the CSS PROFILE or University Financial Aid Application does not match. This form may also be requested in order to confirm the identity of the family members reported in the household size and/or number in college on an application.

Household Support Form -- (Completed by a Dependent Student and the Parent or by an Independent Student)

Independent Document of Support Form -- (Completed by an Independent Student) Use this to document the support given to additional members of your household (e.g. child).

Interest and Dividend Income Form -- (Completed by a Parent of a Dependent Student or by a Student) This form should be submitted to verify investments which generated interest and/or dividend income.

January Term Direct Parent PLUS Loan Application Form -- (Completed by the Parent) Use this form to request a Direct Parent PLUS loan for January Term 2012. You must also complete the federal application at studentloans.gov.

Non-Custodial Parent Statement -- (Completed by a Dependent Student's Non-Custodial Parent) This form should be submitted when providing information on a student's non-custodial parent.

One-Time Computer Expense Request Form -- (Completed by Dependent or Independent Students) This form should be submitted when requesting an increase to the Cost of Attendance for the purchase of a computer.

Parent Estimated Income Form -- (Completed by a Parent of a Dependent Student) This form should be submitted when requesting an appeal for reconsideration of household income loss.

Parent Non-Tax Filer Statement -- (Completed by a Parent of a Dependent Student) This form should be submitted when the parent(s) was not required to complete a federal tax return and/or to report untaxed income earned.

Parent and Student Signature Page -- (Completed by the Student and the Parent of a Dependent Student who has completed the University Financial Aid Application or the CSS/PROFILE online.)

Preparatory Coursework Certification Form -- (Completed by the Student) The Department of Education requires that Student Financial Services verify that students participating in preparatory coursework have not previously participated in preparatory coursework using Federal financial aid and that the student’s coursework is necessary for admission to a Post Baccalaureate program of study.

Satisfactory Academic Progress Appeal Form -- (Completed by the Student) Use this form to appeal for reinstatement of financial aid after suspension for failure to maintain Satisfactory Academic Progress.

SNAP Benefits Received Form -- (Completed by the Student and the Parent) Generally, the SNAP Benefits Received Form is requested because you reported that you or someone in your household received Supplemental Nutrition Assistance Program or SNAP benefits (formerly known as food stamps) at any time during the 2010 or 2011 calendar year on the Free Application for Federal Student Aid and/or University Financial Aid Application.

Student and/or Spouse Estimated Income Form -- (Completed by the Student) This form should be submitted when requesting an appeal for reconsideration of household income loss.

Student Non-Tax Filer Statement -- (Completed by the Student and/or Student's Spouse) This form should be submitted when the student and/or student's spouse was not required to complete a federal tax return and/or to report untaxed income earned.

Study Abroad Cost List -- (Complete by the Student) Use this document as a fax cover sheet for the documents associated with the costs of your study abroad program.

SUMMER 2013 Direct Parent PLUS Loan Application Form -- (Completed by parent of a dependent student) This form must be submitted when applying for a PLUS loan for Undergraduate Students.

SUMMER 2013 Financial Aid Application Form -- (Completed by the Student) Use this form to apply for need-based financial aid for any Summer 2013 academic session at the University of Virginia. Aid is not available for courses taken at another institution.

SUMMER 2013 Study Abroad Cost List -- (Completed by the Student) Use this document as a fax cover sheet for the documents associated with the costs of your study abroad program.

TEACH Grant Request Form -- (Completed by the Student) Use this form to apply for a TEACH Grant.

Third-Party Payment Form -- Use this form to report pending Veteran's benefits, outside scholarships, or other third-party forms of payment.

Trust Information Form -- Requested when it appears that you and/or your parent(s) have one or more trusts. To proceed with determining your financial aid eligibility you must submit documentation of the trusts. Please review the Trust Information Form for information about what you are required to submit.

Unsubsidized Loan Increase Request Form -- (Completed by the Student) In the event a parent is declined for a Parent PLUS loan, this form is used to indicate a request to have the amount of the unsubsidized loan increased accordingly, if the student has filed a FAFSA for the year.
