Student ID Card
It is important to report a lost or stolen University ID card immediately online. You may immediately reactivate it by going back online.
OR
You may call the ID Card Office at 434-924-4508 Monday - Friday 8:00 a.m. - 5:00 p.m., or email. Please include your complete name, University computing ID, and ONLY the last 5-digits of your University 9-digit ID# (if known) for security purposes.
After hours and weekends: If your University ID card is lost or stolen and you need access to your residence, call Systems Control at 982-4685.
You are responsible for any account activity made prior to reporting the loss of your card. By immediately reporting your University ID card as lost, your Cavalier Advantage funds, Meal Plan, Plus Dollars, and privileges, etc., are protected.
If you find someone else's card, please notify us and bring the card to our office.
Replacement ID cards may be obtained at the ID Card Office, Monday - Friday 8:00 a.m. - 5:00 p.m., for a fee of $15, and may be paid for by using cash, check or Cavalier Advantage. We are NOT able to accept credit cards, debit cards or student charge. Payment MUST be made before the card is able to be produced, and the fee is nonrefundable. Cards replaced due to a malfunction or a name change are issued at no charge provided the old card is returned at the time of service.
A Student ID Card Application Form will have to be completed and signed by the student in the office. In addition, another form of a valid government issued picture identification card is required. (Either your U.S. Driver’s License issued by DMV, U.S. Identification Card issued by DMV, U.S. Permanent Resident/U.S. Department of Homeland Security/U.S. Green Card, Passport book or U.S. Military ID Card. Note: Driver's License's stamped "void" and Passport cards are unable to be accepted per Homeland Security.) Information on obtaining your U.S. Driver's License or U.S. Photo Identification card from DMV.
The student's information will be verified in the University's Student Information System (SIS) prior to issuing the ID card.
The student being issued the ID is the ONLY one allowed to use or pick up the card. ID cards are never mailed.

U.Va. Health System Employees
To report a lost or stolen U.Va. Health System ID Badge, contact the U.Va. Health System Technical Services at 434-982-4009 or by email. They are located in Room M1205 in the front corridor of the Multistory of the West Complex (old Hospital).
