**Important Notes – please read:
- Form #1 must be submitted for NEW campers only (available below).
- Forms # 2, 3 and 4 must be submitted for ALL campers at the time of registration (available below).
- Printed materials and forms are available below (PDFs) and in the IM-Rec Sports Business Office at the Aquatic & Fitness Ctr. Call 434.924.3791 or e-mail us if you need assistance or have questions.
- All forms with non-refundable deposit must be submitted in-person at the TIME OF REGISTRATION.
- Please do NOT mail, e-mail, or FAX any registration materials; they will not be processed.
