Step 1: Complete the Forms on our Registration Forms Page.

All families must complete the "Camper Registration Form" (CDC Form 2), the "Medical Information Form" (CDC Form 3) and the "Sunscreen Authorization Form" (CDC Form 4). If your child attended Cavalier Day Camp in 2007, you will not need to re-submit the "Physical Examination and Immunization Certification" form (CDC Form 1). ALL forms MUST be completed and turned in at the time of registration.

Step 2:
Register Your Camper!

UVA IM-Rec Sports Members:
"Cavalier Day Camp Member Registration Event"
Monday, March 17, 6:00–7:00 PM, in the Aquatic & Fitness Center
gymnasium. Sign-up will be first-come, first-serve. Parents will have the opportunity to meet members of the camp staff and reserve a spot in Cavalier Day Camp 2008. Registrations will be accepted after this date on a space availability basis only. Priority registration for members will continue through Friday, March 21 in the Aquatic & Fitness Center Business Office (Mon–Fri, 8:00 AM–5:30 PM). All deposits and forms must be submitted at the time of registration. To take advantage of the benefits of IM-Rec Sports memberships, members must be the parent or legal guardian of the child that they are enrolling. In addition, memberships must be current at both the time of registration and during the camp session a child is enrolled in.

Open Registration
Members and non-members may register for camp beginning on Monday, March 24 and anytime thereafter on a space availability basis only in the Aquatic & Fitness Center Business Office (Mon–Fri, 8:00 AM–5:30 PM). All deposits and forms must be submitted at the time of registration.

Step 3: Provide Documentation of your Child's Age at Time of Registration.

As a licensed day care center, we are required by state law to verify the age of your child. Please bring a copy of your child's birth certificate when you register for our records. If registering a camper that is not your child, you must provide legal documentation of guardianship at the time of registration. If your child attended Cavalier Day Camp in 2007, you will not need to re-submit a copy of your child's birth certificate.

Step 4: Submit Payments at Time of Registration.

Deposits: A non-refundable, non-transferable deposit of $50 (per camper) will be required for each session that you enroll a camper and is due at the time of registration. This deposit will be applied to the session rate. Please plan your child's summer schedule carefully in order to avoid forfeiting your deposit. Once your camper is registered, a detailed payment schedule including the dates and the amount of money due will be e-mailed to you.

Cavalier Day Camp Expenses Eligible for Flexible Reimbursement Accounts:

  • As a licensed day camp provider by the Commonwealth of Virginia's Department of Social Services, the Cavalier Day Camp's registration fees charged to participants qualify as one of the two types of reimbursement accounts that are exempt from personal income tax for all UVa employees EXCEPT hourly wage employees not receiving benefits.

  • Employees that have set aside money to pay for dependent care expenses for children entering the first through fifth grades are eligible to be reimbursed for CDC expenses through the Flexible Reimbursement Plan. To participate in summer 2008, an employee must have already selected an annual contribution amount that is payroll-deducted, before taxes are calculated, during the plan's open enrollment period earlier this year. New enrollment forms are required every year during open enrollment to take advantage of this benefit.

  • For more information or for reimbursement form requests from the University Human Resources Benefits Division, employees should call 434.924.4392 or e-mail benefits@virginia.edu.
