Call 434.924.3791 today for a membership application, or download it here. Complete the form by printing clearly and including all the information requested. Please be sure to include your check, made payable to UVA Intramurals, a completed payroll deduction form, or your VISA/MasterCard or Discover number & expiration date. Send your materials to us:

1.	By Mail!

2.	By Fax!
	Fax your application to 434.924.3858. If you use payroll deduction, you can fax your membership application & payroll deduction forms to us at 434.924.3858.

3.	In Person!
	Drop by the Intramural-Recreational Sports Business Office in Room 209 of the Aquatic & Fitness Center during our business hours: Monday–Friday, 8:00 AM–5:30 PM. We'll be happy to help you!

Plus One memberships must be purchased in person. *Proof of age and verification of residence of the Plus One* member will be required** as part of the Recreational Sports membership application process.The faculty/staff/student member sponsoring the Plus One member must be present at time of purchase! Accepted Documents for Proof of Residence, required for both parties, include: Valid Department of Motor Vehicles Identification (i.e. driver's license) Valid state or government issued license (for personal or business) Printed lease with both tenant & landlord signatures (no handwritten documents please!) Voter registration card Top portion of federal or state income tax form, showing name & address Utility bill with the name & address of Plus One applicant. Learn more about Plus One memberships.

I.D. Cards:

We will activate faculty/staff valid UVA i.d. cards within 24 hours, and if you mailed or faxed your membership application, we'll send you written confirmation of your membership by mail. Spouses and family members (dependents ages 18–23) will be issued an IM-Rec Sports photo i.d. membership card (to be issued at the IM-Rec Sports Business Office). Membership cards for children under age 18 do not require a photo i.d.

However you choose to purchase your membership, we strongly encourage you to drop by our Business Office in the Aquatic and Fitness to learn more about your new recreation and fitness opportunities at IM-Rec Sports.
