Purpose

To designate employees in each department who will control and
distribute paychecks.

Instructions

The numbered items on the Designation of Payroll Distribution
Officers (UT-181) sample form should be completed as follows:

 1.  Effective date of this form.

 2.  Payroll department name or name of direct deposit financial
     institution.

 3.  Three letter Home Department Code representing this payroll
     department.

 4.  Department address.

 5.  Department telephone number.

 6 - 10  These items are for each of three designated people.

 6.  Name of designated Payroll Distribution Officer.

 7.  Position of Payroll Distribution Officer.

 8.  Office telephone number of Payroll Distribution Officer.

 9.  Signature of Payroll Distribution Officer.

10.  Current date.

11.  Name of official signing this form.  PLEASE PRINT OR TYPE IN
     THIS SPACE.

12.  Position of official signing this form.

13.  Signature of official.

14.  Current date.

PLEASE NOTE DISPOSITION BELOW.

NOTE:  All employees distributing payroll checks must be listed on
a properly executed UT-181. This form should be updated every six
months and/or whenever there is a change in Payroll Distribution
Officers.

THE PAYROLL DISTRIBUTION OFFICERS AND DEPARTMENTAL PAYROLL CLERKS
(EMPLOYEES WHO PRODUCE INPUT TO THE PAYROLL SYSTEM) MUST ALWAYS BE
DIFFERENT PEOPLE.

Each payroll department should have at least two Payroll
Distribution Officers.  The Designation of Payroll Distribution
Officers form has room for three names.  Additional pages may be
used if more than three Payroll Distribution Officers are needed.

Part-time employees or non-employees should never be permitted to
distribute payroll checks to full-time employees.  In cases where
part-time employees supervise other part-time employees, the part-
time supervisors may distribute payroll checks to part-time
employees with the approval of their full-time supervisor.

Disposition

White Copy  -- University Departments - To Check Distribution
               Section, Bursar's Office.

               Hospital Departments - To Hospital Payroll Office.

Canary Copy -- To Regional Payroll Distributor.
