Checklist for Planned Disability Leave (Including Childbirth)

Initiate conversation about your potential leave with your supervisor. For teaching and research faculty, the supervisor is typically your department chair, a division chief, or a dean. (NOTE: While it is in your best interest to discuss your upcoming absence with your supervisor, you do not need to reveal any medical or personal details regarding the reason).

 

File a Claim with The Standard based on the date of your planned absence (due date, surgery date, etc.) at 1-800-378-2395. The Leave Center of the University’s Benefits Office will contact your supervisor after you file the short term disability claim.

Your department is responsible for coordinating arrangements for covering responsibilities, including teaching and research, after The Standard has made a decision regarding your absence.

 

If planning to return to work prior to the end of your approved leave time, contact The Standard who will notify your department to make arrangements.
