Participating students who enroll in the health insurance plan may enroll in an insured dental program through United Concordia at the annual costs shown below. The annual maximum benefit is $750 per insured.

What deductibles, coinsurance, and exclusions are included in the dental plan?
The annual deductible is $50 per enrollee. There is no coinsurance for Class I services (e.g., exams, x-rays, cleanings); basic restorative work is paid at 80% of allowable costs; and crowns, inlays, orthodontia, and prosthetics are excluded from coverage.
