How to Meet University Requirements

What is "Appropriate Documentation" for mumps?
Students must submit appropriate documentation of mumps vaccine to Student Health, as required by Virginia state law, or the University will place a registration block on course enrollment until the requirement is fulfilled.
Students must have one of the following:
- Pre-Entrance Health Form completed documenting 2 MMR vaccines
- Immunization Record by family doctor/clinic documenting 2 MMR vaccines
- Immunization Record by family doctor/clinic documenting positive blood test for mumps ("titer")

What is YOUR Mumps Documentation Status?
use these guidelines to determine your status:
If you:
- Received an email from Student Health
- Have a block on your class registration
Then you:
- Have not met the University and Virginia State law requirements, for one of the following reasons:
I am unsure why I received the email
Call the U.Va. Student Health Mumps Hotline (434) 924-1525 to ask if your pre-entrance health form is updated with the required two (2) MMR (mumps, measles, rubella) vaccines. If it is not, follow Medical Records staff instructions.
I have not turned in my health form or immunization record
- All University of Virginia students must obtain their required immunizations, as stated by Virginia state law, and all students are required to turn in their completed pre-entrance health form to Student Health no later than August 1st. Students who do not meet the Virgina state law and University requirements before the end of the grace period (late September) will have a block put on their registration by the University until they meet the requirements.
- To take off the registration block, you need to do one of the following:
a. Complete your pre-entrance health form
b. Obtain your immunization record from your family doctor
c. Receive your MMR vaccine(s)
- The pre-entrance health form and/or immunization record may be
faxed to Medical Records at:
(434) 982-4262, or bring them to Student Health between 8am-5pm, Monday-Friday.
- If you are unable to obtain the appropriate documentation, you will need to come to Student Health to receive your MMR vaccine. MMR vaccines are administered Monday-Friday, 9am-12pm and 2pm-4pm in the General Medicine clinic.
- Call the U.Va. Mumps Hotline (434) 924-1525 if you have questions.
I have not received one or both of my MMR vaccine(s)
- All University of Virginia students must obtain their required immunizations, as stated by Virginia state law, and all students are required to turn in their completed pre-entrance health form to Student Health no later than August 1st. - You
can obtain your MMR vaccine(s) (administered tol University of Virginia students only) on a walk-in basis in the General Medicine clinic at Student Health:

I have signed an exemption form (religious or medical)
- If you currently have a religious or medical exemption, you have an option of receiving a blood test ("titer") which measures mumps antibody levels, and can determine whether you are immune to mumps. Call the U.Va. Mumps Hotline (434) 924-1525 for more information.
- Please note: two cases of mumps (as of 03/10/2008) have been identified in U.Va. students. Although efforts are on-going to minimize exposure, there is always a risk of a larger outbreak on a college campus.
- If you have a medical exemption from mumps vaccination, we urge you to consider consulting with your personal physician or a physician at Student Health to re-evaluate your situation.
