For concerns related to general healthy eating, initial appointments are conducted by a PHE. For all other concerns, initial appointments are conducted by the dietitian. Initial appointments last for 1 hour and are conducted individually.
Once you schedule your initial appointment, you will need to log into https://www.healthyhoos.virginia.edu and complete the “Nutrition Questionnaire”. You will also need to bring a completed Food Log form to your appointment. During this initial session we will begin an assessment, discuss nutrition recommendations, and help you develop a plan to achieve your nutrition goals.
After your initial appointment, you are eligible to schedule 25-minute individual follow-up appointments with the dietitian.
If you have questions, please call 924-1509.
