High School Students Application Forms


Carefully read the following instructions before downloading the High School Application.

  1. High School applicants must complete all personal information (pages 1-2).
  2. Secondary School Report (page 3) is completed by the guidance counselor or principal of the High School currently attended by the applicant. Remember to include name on top of the form.
  3. Medical Consent Form for Minors and Terms of Understanding for Parents of High School Students (page 4) must be completed by the parent or legal guardian of high school applicant.
  4. The University of Virginia Honor System Pledge (page 5) must be signed by the high school applicant.
  5. Health Insurance Verification (page 5) must be signed by the parent or legal guardian of the high school applicant.
  6. The Legal Disclosure Statement (page 6) must be signed.
  7. Review the UVa nondiscrimination policy (page 6).
  8. Review the Family Education Rights and Privacy Act (FERPA) statement (page 6), and the FERPA Authorization to Release Financial Information form (page 7).
  9. If the high school applicant is a Virginia resident for in-state tuition purposes, the Application for Virginia In State Privileges must be completed by both the applicant as well as their parent or legal guardian and be returned with the high school application.
  10. Complete the checklist (page 8).

Remember to reset the form after printing so that your personal information will be cleared.
