Daren Anderson

Daren Anderson, MD, Chief Medical Officer, Community Health Center, Inc., Middletown , CT and Principle Investigator, RWJ Project for Advancing Diabetes Self-Management

Dr. Anderson is a general internist and the Chief Medical Officer for Community Health Center, Inc, a large, multi-site Federally Qualified Health Center in Connecticut serving medically underserved patients across the state. CHC has been a national leader in primary care quality improvement with successful initiatives including the Chronic Care Model, Advanced Access Scheduling, integrated mental health, and comprehensive self management for diabetes. Dr. Anderson is a well-recognized speaker on these and other community health issues.

Dr. Anderson has served as the senior leader for CHC's teams participating in the Bureau of Primary Care's Health Disparities Collaboratives in diabetes, asthma, cancer screening, and clinical redesign. In addition, Dr. Anderson is the Principal Investigator for the RWJ Diabetes Self Management grant. Other projects he is involved in include initiatives focused on obesity prevention, diabetes prevention, and Planned Care. Dr. Anderson is a consultant for Health and Technology Vector, a medical consulting firm specializing in the field of disease management. His clients include Healthways, the largest Disease Management firm in the US .

Dr. Anderson is a graduate of Harvard College and obtained his medical degree from the Columbia University College of Physicians and Surgeons in New York City . He completed his residency in internal medicine at Yale- New Haven Hospital and has worked as a general internist in primary care in New Britain , CT.
