Protocol and Consent Form Additions
Protocol:
- If your research focuses on at-risk children who may be more likely to suffer from abusive situations, you will need to demonstrate your experience in working with this population, which can be done in 8c.
- In 11, clearly explain how the data will be collected and stored; it is possible that you will need a Certificate of Confidentiality if your research might reveal information about illegal behaviors. Please see the Certificate of Confidentiality section for more information.
- In 21, give specific detail about the risks faced by the participants and the procedures you will have in place to deal with possible harm to the participants.
Consent Form
As your study will involve a child, you will need to provide a consent form for the parent(s) and an assent form for the child. Please use the Child Assent/ Parent Consent Template to assist you in creating these forms. For more information about parent consent/ child assent, please see Education: Consent: Parent Consent.
Reporting suspicions of child abuse requires a breach of confidentiality. If the methods used in your research increase the likelihood of discovering cases of abuse, the Board will ask that you add the following statement to your consent form and assent form under “Confidentiality”:
Consent form (please modify so that it is appropriate for the participants’ reading level): I have ethical and legal obligations to report suspected child abuse or neglect and to prevent your child from carrying out any threats to do serious harm to themselves or others. If keeping information obtained in this study private would immediately put them or someone else in danger, I will release that information to protect them or another person.
Assent form (please modify so that it is appropriate for the participants’ reading level): If I think that someone is doing bad things to you, or if I think you are doing bad things to yourself or to other people, I will need to tell someone about it so that I can help you.
Please note: If you are using a Certificate of Confidentiality, you may be required to include specific template language that covers what is described above. Please provide a consent form that is complete, concise, and covers all necessary information without being redundant. The Certificate of Confidentiality section provides more specific information on template language that needs to be added to your consent form.
