Protocol and Consent Form Additions
Protocol:
- If your research focuses on at-risk adults who may be more likely to make threats or be threatened, you will need to demonstrate your experience in working with these participants, which can be done in 8c.
- In 11, clearly explain how the data will be collected and stored; it is possible that you will need a Certificate of Confidentiality if you are recording information about illegal behaviors. Please see the Certificate of Confidentiality section for more information.
- In 21, give specific detail about the procedures you will have in place for reporting a threat.
Consent Form:
Reporting a threat requires a breach of confidentiality. If the methods used in your research increase the likelihood of discovering of a threat, the Board may ask that you add the following statement to your consent form under “Confidentiality”:
Consent form text (please modify so that it is appropriate for the participants’ reading level):
I have ethical obligations to report suspected abuse and to prevent you or others from carrying out threats or doing serious harm. If keeping information obtained in this study private would immediately put your and/or another person in danger, I will release that information.
Please note: If you are using a Certificate of Confidentiality, you may be required to include specific template language. Please provide a consent form that is complete, concise, and covers all necessary information without being redundant. The Certificate of Confidentiality section provides more specific information on template language that needs to be added to your consent form.
