Materials Release Form

If you are using your recorded data beyond the data analysis, you may consider using a Materials Release Form so that the participant can consent to these procedures separately from the study consent.  For example, if you intend to use a video recording in a conference presentation or classroom instruction, the participant will need to consent to this use of their data.  The participant may be willing to participate in the study, but they may not want to have their data used for other purposes. Using a Materials Release Form will help to divide the two procedures for the participant. Please see Materials Release Form for more information about how to construct your consent procedures.
