The IRB-SBS has a growing collection of commonly used instruments (surveys, exams, etc.) that we have received through various submissions. In order to relieve researchers from repeatedly submitting the same instruments, the IRB-SBS has created the Instruments Library. If you are using any of the instruments listed below, it is not necessary for you to submit a copy of the instrument with your protocol submission. Instead, list the Instrument Number under 9b of the protocol (make sure you are using the most recent version of the protocol form). If you would like to add an instrument to our library or update an existing instrument, please send the electronic version of the instrument to Katie Rees at katierees@virginia.edu, send a hard copy to our office, or make note of it in your protocol submission.
