Quick Guide to Consent Templates
- General Consent Template: Basic guide for creating most informed consent agreements.
- Parent Consent/ Child Assent Template: Guide for creating informed consent and assent agreements for parents and children. The template includes a parent consent form and assent forms for older minors and younger minors. This form can also be used as a guide for surrogate consent procedures.
- Parent Notification Template: Guide for creating a notification letter to send home to parents. This form is typically used in normal educational practice studies where the study is deemed exempt, though it may be appropriate for other situations as well.
- Debriefing Form(s): This form is typically used in studies involving deception and/or studies that include participants from the Psychology Participant Pool and the Curry Participant Pool. Please see Debriefing Participants.
- Post-Debrief Consent Form(s): This form is used in a deception study after the deception is revealed to the participant. The participant is given an opportunity to decide if they still want to participate after the true purpose of the study is revealed.
- Data Release Form(s): This form is similar to the Post-Debrief Consent Form; it is used when a participant has been recorded or photographed without their knowledge.
- Materials Release Form(s): This form allows a participant to declare how they would like their materials to be used by the researcher if the researcher wants to use the materials in situations beyond the study, such as archiving the materials in a library or using the materials for classroom exercises.
- Alternative Consent and Risk Reduction Procedures: This document provides a template for oral consent as well as information about international and ethnographic studies.
