The Major Declaration Worksheet form is filled out when a prospective Cognitive Science major meets with the Program Coordinator. It is meant to be used as a planning document and does not commit the student to taking the courses selected on this form. This sheet is simply a means for students to map out the courses needed to complete the Cognitive Science Major.

The list of Regularly Offered Courses should be used along with the Major Declaration Worksheet to plan students' courses for completing the Cognitive Science Major.

The Computer Science Concentration form is required for all cognitive science students who plan to concentrate in Computer Science thru the Engineering School. It must be completed and signed by someone in the Computer Science department before declaring in the major and for use during advising.

The Double Major with Arts & Sciences Interdisciplinary Computer Science form is used with students double majoring in Cognitive Science and the interdisciplinary Computer Science Program in the School of Arts & Sciences. It is filled out with the Program Coordinator at the time of declaring a major in Cognitive Science and will be used for advising. The requirements for both majors overlap, and this form is necessary for insuring completion of both programs.

The Double Major with Psychology form is used with students double majoring in Cognitive Science and Psychology. It is filled out with the Program Coordinator at the time of declaring a major in Cognitive Science and will be used for advising. The requirements for both majors overlap, and this form is necessary for insuring completion of both programs.
