Mailing Instructions

Mail (1-x) copies according to sponsor requirements. If there are no sponsor requirements, send three (3) copies.


Notes:

Note 1: Format used for proposals which do not have specific guidelines and are not submitted to industry.

Note 2: If the document is stapled, center cover page in a table.  Most letter proposals will not have a cover page but will begin with a letter stapled to the remaining proposal with signature lines for PI, Chair and OSP..

Note 3: Every proposal must have the proposal no. appearing on the front cover of the document for tracking purposes.

Note 4:  Contractual Information section is needed only if there has not been a form provided within the proposal for the address and the signature of the Director of Sponsored Programs (Mr. D. W. Jennings).  All proposals submitted must have this information.  Letter proposals do not need a contractual information section if the address and signature of the Director of Sponsored Programs is included on the letter of submission.
